The intake form will be resent by email, and the form link will be included in the email. We kindly ask that you complete it before your next treatment visit.

If the schedule is fully booked for the day, you are welcome to text or email us to inquire about an additional acupuncture session — we have independent tables available to help accommodate extra appointments when possible.
